Case Studies in Abdominal and Pelvic Imaging is a collection of 100 real cases,
encompassing a broad range from common medical and surgical problems to more
rare but interesting pathologies.
The cases demonstrate the use of modern imaging techniques that are generally
commonplace to most hospitals, and illustrate how multiple modalities can be used in
the investigation of pathology.
The advent of PACS has meant images are more readily available to clinicians for
review in clinics, wards, etc. With the increasing number of multi-disciplinary meetings,
the imaging of more cases is reviewed and demonstrated by radiologists to a
wider group of clinicians. It is therefore important to have an understanding of imaging
and not simply to read the report.
In addition more US is being performed as an extension of examination and a
good understanding of the appearances and pathologies that may be demonstrated by
this clinician-performed US is essential.
This book is aimed at a broad range of specialties including gastroenterology,
general and GI surgery, gynaecology and urology and also radiologists in training and
medical students.
In addition, we intend its use to extend to those allied professionals who regularly
review imaging when treating their patients, e.g. cancer specialist nurses and
endoscopists.
The structure of the book is designed to enable the reader to study 100 cases. Each
commences with a brief history, accompanied by the relevant images and questions
on one page. The answers to the questions are found on the next page, with annotated
images demonstrating the salient features. There is then a brief discussion of the
condition, key teaching points, references and suggested further reading. This format
reflects changes in medical education, where some of the more traditional formats
have been replaced by clinical scenarios which often include an element of imaging.

خلاصه

چشم از راههای مختلفی می تواند دچار آسیب شود . تروما بعنوان یکی از علل کاهش دید و دوبینی و فلج زوجهای حرکتی چشم مطرح است . یکی از علل ترومای چشم و آسیب بینایی نیز همراهی ضربه مغزی با آسیب به چشم می باشد . هدف ما از این مطالعه شناخت بیشتر شیوع فلج زوجهای حرکتی چشم در افراد دچار ضربه مغزی و ضربه به سر بوده است .

این مطالعه از نوع توصیفی – تحلیلی و به روش Case Series  انجام گرفته است که در بهار و تابستان 1385 و بر روی 300 نمونه از بیماران ضربه مغزی و بستری در بخش جراحی مغز و اعصاب بیمارستان شهید رهنمون یزد صورت گرفته است .

میانگین سنی جامعه مورد بررسی 46 سال با محدوده سنی 1 تا 87 سال بوده است . 242 نفر (1/81%) از آنها را مردان و 58 نفر (9/18%) را زنان تشکیل می دادند در مجموع 7 نفر دچار فلج زوجهای حرکتی چشم بودند که 5 نفر از آنها (71%) مرد و 2نفر (29%) زن می باشند .

در گروه سنی 14-1 سال ، شیوع ضایعات فلجی بیشتر بوده است .

در بررسی توزیع فراوانی وضعیت بینایی بیماران و مشکلات چشمی ، دوبینی از همه شایعتر بود(6/6%) از کل بیماران بستری شده 202 نفر سطح هوشیاری طبیعی داشتند (3/67%) و تنها 8 نفر GCS کمتر از 8 داشته اند (7/2%)

از نظر فلج اعصاب حرکتی چشم ، عمده درگیری عصبی در زوجهای 4 و 6 دیده می شود . در مجموع 7 نفر از بیماران فلج پایدار عصبی پیدا کرده اند . و 23 نفر دیگر فلج گذرای اعصاب حرکتی چشم را نشان داده اند .

بیشترین آسیب وارد شده مربوط به هماتوم ساب دورال و ساب آراکنوئید است که هرکدام در 9/22% موارد دیده می شود .

ارتباطی بین سن و جنس و نوع فلج عصب نیز وجود نداشت .

Conclusion : هر چند فلج اعصاب کرانیال در ترومای به سر خیلی شایع نمی باشد لیکن لزوم توجه به معاینات چشم پزشکی در موارد ضربه مغزی وجود دارد از طرفی با توجه به عدم شناخت دقیق علل منجر به ضایعات حرکتی چشم نیاز به تحقیقات وسیعتر بعدی می باشد .
